Understand your health. Discover your patterns.
Your health is influenced by more than your diagnosis. HealthTracker helps you connect your treatment, daily symptoms and real-world experiences to understand what may be helping, what may be making things worse, and what to discuss with your healthcare team.
Record what happened. Track how you felt. Discover what may be connected.
Knowing your diagnosis is not the same as understanding your days.
You may know your condition and your treatment and still wonder why some days are so much harder than others. HealthTracker brings your records together instead of leaving them scattered across notes, diaries, apps and memory.
Your experience can be shaped by
- Medication and treatment changes
- Sleep, fatigue and activity
- Stress and emotional wellbeing
- Food, drink and daily routines
- Weather, allergens, damp, pollution
- Illness, infections and other symptoms
- Work, travel, social activities and life events
- Other factors specific to your condition
How HealthTracker works
Select your condition
Choose from the conditions library or add your own. HealthTracker suggests symptoms and factors people commonly record. They are starting points you can edit, because no two conditions, or people, are the same.
Add your care plan and treatment
Enter what your GP, consultant or pharmacist has prescribed or advised, with dates, and record changes as they happen. Prescribed treatment is kept separate from anything you take or do yourself.
Check in each day
One overall rating from 1 to 5, a few optional symptom ratings, and a tick for anything that was relevant. About a minute. Use Quick log to note things as they happen.
Discover your patterns
Charts and careful comparisons appear as your days build up, showing what you recorded, how many days they rest on and what they cannot tell you.
See what may be connected.
Wellbeing, symptoms, treatment changes and daily events sit on one timeline, so you and your healthcare team can look at them together.
Examples of how observations are worded
- Your symptoms have tended to be worse on days when you recorded poor sleep.
- You reported increased symptoms on several occasions following this recorded exposure. There are also days when the exposure occurred without a reported increase.
- Your wellbeing improved during the period after this treatment change, although other factors may also have contributed.
- Not enough information is available yet to identify a reliable pattern.
HealthTracker never says something caused something else just because they happened together.
Built to be careful with your information
Health information is sensitive. HealthTracker collects as little as it needs, and keeps you in control.
- Your free-text notes are encrypted before they are stored.
- Download everything you have recorded, or delete your account and all its data, from Settings at any time.
- Reminder emails never contain health information.
- You share a summary only by creating a link yourself. It expires after 7 days and you can switch it off.
- No advertising and no tracking scripts.
- A personal tracking tool, not medical advice. It does not diagnose or recommend changing treatment, and it is not connected to NHS records.
For healthcare professionals
Patients can prepare a concise summary of their ratings, symptoms, treatment history and questions before an appointment.
It is patient-recorded information, clearly labelled as such. See what a summary contains.
