For healthcare professionals
Patients can bring a concise summary of what they have recorded between appointments. It is patient-recorded information, labelled as such.
What a summary contains
- The condition or conditions the patient is tracking, and the period covered.
- Daily wellbeing ratings (1 to 5) with a 7-day average, and how complete the record is.
- Symptom ratings the patient chose to track, summarised by days recorded, average and highest.
- Treatment history: what the patient says was prescribed or advised, and anything they take themselves, each marked as such, with start dates, stop dates and recorded changes.
- Whether doses were recorded as taken, late, missed or changed.
- Events and exposures the patient recorded, by number of days.
- Descriptive comparisons, such as average wellbeing on days with and without a recorded factor, always with the number of days involved.
- Questions the patient would like to ask.
How to read it
- Everything is entered by the patient and has not been verified. Doses and prescriptions are as the patient typed them.
- Pattern comparisons are descriptive. They do not establish cause, and they can arise by chance, especially with few days.
- HealthTracker is not connected to NHS or other clinical records, and nothing is sent to a clinician automatically.
How a patient shares it
The patient can print the summary or save it as a PDF, or create a private link that expires after 7 days and that they can switch off at any time. Please treat anything shared with you as confidential health information.
