About HealthTracker
HealthTracker connects four things people usually keep apart: what their healthcare team has advised, their treatment, how they feel each day, and what is going on in their life. Looking at them together can make it easier to see what may be helping, what may be making things worse, and what to talk about at an appointment.
What it is
A personal diary and summary tool. You record, and it shows you charts and careful comparisons of your own records.
What it is not
- It is not a diagnosis, a prediction or medical advice.
- It never tells you to start, stop or change a treatment. Decisions about treatment belong with you and your prescriber.
- It is not connected to NHS records or any clinical system, and it is not an NHS service.
- It cannot show that one thing caused another. Two things happening together in your records can be coincidence, or both can be caused by something else.
- Nobody monitors your entries, so it cannot notice that you are unwell.
If you feel unwell
In an emergency call 999. For urgent medical advice in the UK call NHS 111 or contact your GP. If you are having thoughts of harming yourself, call 999 or contact Samaritans free on 116 123.
Do not wait for a chart or a pattern before getting help for symptoms that worry you, such as severe breathlessness, chest pain, sudden weakness, confusion, or a rapid worsening of your condition.
Where the suggestions come from
The symptom and factor suggestions for each condition are general starting points for self-tracking. They have not yet been clinically reviewed and you can edit them. For medical information use NHS and NICE resources and your healthcare team.
